Synergy Care is looking for a qualified Occupational Therapist/Rehab Director for an open position at a luxury Independent/Assisted Living and Memory Care in Houston, TX. We are looking for someone who has a minimum of 2 years in AL/IL communities.
Job responsibilities:
Provides occupational therapy and related services for patients, caregivers and facilities in accordance with the principles and practices of occupational therapy and with Synergy Care's guidelines. Provides quality OT including: Evaluation of patients, development of initial and ongoing treatment & discharge plan, supervision of assistants and aides & participates in case management. Represents Synergy Care's commitment to quality & service in all interactions with facility. Adheres to all state regulations regarding occupational therapy.
